BBC News field order problems

Has anyone else noticed at least the last couple of nights have had field order problems in some of their news packages. Wednesday night news at 10 had a piece about commuter satisfaction where there was some footage shot out of a moving train, there was a significant issue with this material, I can't believe the editor did not notice and correct the problem before sending it to TX. Thursday also had a package that had similar issues. Do the BBC not check anything now before airing it?
