I can only really think of the rings of akhaten?
Please list where and when he references it please

'The Beast Below' (in a conversation with Amy)
'The Doctor's Wife' (sort of, with Amy and Rory)
There weren't that many.
Hang on until the 23rd of November and you'll have no shortage of them.
Are we going to count the dialogue introducing Hurt provisionally? It wasn't explicit and Moffatt is tricksy but I don't think it's controversial to say that it was clearly meant to conjure the war.
There also have been some last of his kind mentions, for example in Vampires of Venice and The Hungry Earth for example.
But I'm also glad that he seems to have moved on from the whole thing a bit since Ten, which makes sense especially as it now has been over 300 years ago.
I mean he obviously hasn't forgotten it as he still brings it up sometimes and it still influences some of his actions, but IMO there's no need for him to bring it up every episode.
"I marked the passing of the Time Lords..."
Umm, it's already been mentioned twice, including by the OP.
Jo brings up the Time Lords, and The Doctor just lets it go, but you can see by the look on his face he doesn't want to tell her what happened.
Ha ha! I've been watching too much Cheers lately. I didn't automatically correct the typo.
D'oh!
Do you mean this?
"An ancient creature, drenched in the blood of the innocents. Drifting in space through an endless shifting maze. To such a creature…death would be a gift. It accepted… Then sleep well… I wasn't talking about myself…"
