Try not to worry yourself too much about it. Send her the letter like others have suggested, give her a fairly short (but not unreasonable) time limit, and then pursue further action.
I had to take an employer to court last year to get wages I was owed, and I kept panicking myself thinking it would be really complicated. In the end I sent the letter, she ignored me, so I went ahead with it. She didn't dispute the paperwork she was sent by them and didn't bother turning up for the hearing, both of which worked in my favour - I got what I was owed plus a bit extra for the inconvenience. It wasn't anywhere near as complicated as I had imagined.
