  It's from the Lively Arts documentary.
  The script conference with Dicks, Holmes and Williams was apparently staged specially for the cameras and they aren't talking about any specific story.

  Yes, Terrance Dicks has talked cringingly about this scene a number of times as being wholly fake. The producers of the piece requested an argument or lively discussion between the creators, and they duly staged this debate.
