John Smith came from a brand that the Doctor saw Jamie has from his medical container.... in the episode entitles "The Wheel of Death" Patrick Throughton Era....

I always thought it orginially came from Jon Pertwee era but it actually started with the second doctor
quoted on BBC doctor who site
The Doctor carries lemon sherbets and gets his pseudonym of John Smith from Jamie, who sees it as a brand name on a medical container. [Jamie has learnt to read since The Highlanders.]

As for Mickey, it's just a common surname. Martha's surname was probably so they could use the Smith and Jones pun as the episode title for her debut.

That's not entirely true. RTD has mentioned he is fond of using certain names - Tyler, Jones, Smith, Harkness. One can't claim coincidence when a writer has created characters named Ianto Jones and Yanto Jones, both played by the same actor.

    Oi Newbie, you're back! :)

Think RTD said something once about those 'ordinary' names making him feel comfortable or words to that effect.

The Wheel in Space, in fact. 1968.

Well that explains Ianto Jones then. He was named after Yanto. Still doesn't explain Harriet.

Nothing to explain, really. As long as RTD is writing scripts we're going to be seeing lots of Smiths, Joneses, Tylers, as well as others:

    Century Falls:
    Harkness
    Naismith

    Revalations:
    Tyler

    The Grand:
    Harkness
Bannerman (now only a road, but still)
    Jones
    Tyler

    Queer as Folk:
    Jones
    Tyler
    Smith

    Bob and Rose:
    Smith
    Jackson

    Second Coming:
    Saxon
    Tyler

    Mine all Mine:
    Jones

Besides, making Rose a Tyler was the least he could do for poor Vince after making Paul McGann "count."

You can’t pass your Timelord exam if you can’t count.

Yeah, but what’s the highest number you can count up to?

Some Timelords can count up to the number: 1,872.
