As for Mickey, it's just a common surname. Martha's surname was probably so they could use the Smith and Jones pun as the episode title for her debut.
That's not entirely true. RTD has mentioned he is fond of using certain names - Tyler, Jones, Smith, Harkness. One can't claim coincidence when a writer has created characters named Ianto Jones and Yanto Jones, both played by the same actor.

Well that explains Ianto Jones then. He was named after Yanto. Still doesn't explain Harriet.
