I'd love for it to turn out that The Doctor is a homeless man with a head injury who thinks he's an alien, and has spent the last 50 years living in a telephone box thinking it's a TARDIS and drinking until he thinks he's surrounded by Daleks (postboxes). His adventures include stumbling into a zoo (and meeting the Judoon, Silurians and Sea Devils), blowing up a factory that makes prosthetic (cyberman) limbs, and stalking pretty young women offering to show them his sonic screwdriver.
We see a beach deserted apart from one woman in her mid to late forties, as she walks towards the crashing waves silent tears run down her face crying for the loss of someone close. She is carrying an urn as she gingerly walks into the sea,she no longer feels the cold,she no longer feels anything. Her silent tears become sobbing as she shouts to the sky "why" at that point a noise distracts her, a noise she has not heard for a very long time. She hears a mans voice cry out, she does not need to face him to know who he is. She senses his approach and turns to see the youthfull face of the man she once knew. Again the man speaks "I said I got her just in the nick of time, what on earth are you doing in this water Tegan"
The Doctor becomes fatally allergic to close contact with sonic devices, the last scene in the episode is the Doctor getting Amy to throw his Sonic Screwdriver out of the Tardis.

A rich old woman, whose had children and dozens of grandchildren!
As the meeting progresses Donna says she knows who he is!
She remembered her travels with the doctor, it slowly started coming back to her over the years! Thus not killing her as it happened slowly over time!
She tells him she has had a long and happy and dies in his arms!
The doctor says his farwells and leaves. Not realising that slowly and surely Donna starts to regenerate!
