Going back a bit , the later commissioned Tom and Jerry cartoons.
They may have been politically incorrect in many ways, but the sophisticated animation and early charm of the characters was just lost with the "modern" versions.
Channel 4 cancelling After Dark, the late night "open ended" discussion show.
Introducing boot camps to X Factor, BGT etc - now its about the celebrities trying to promote their own people instead of promoting the best.
Susan Boyle was great example - she didn't win but made more then any other winners!
And, of course, renewing Lorraine Kelly's contract!
X-Factor and BGT have always had bootcamp as part of their audition process. The problem with it is that they've lost track of what Bootcamp should be about, adding twists like the 6 chair thing, and doing it in front of a large audience. Then there are all the twists in the live shows e.g. wildcards and dumping four contestants in week one (which completely backfired on the X-Factor bods when one of the contestants was booted off the show a few weeks later for bad behavior, and they had to then bring back one of the people who was kicked off the show in week one as part of the twist).
There's a lot to be said for keeping things simple and straightforward. All the ridiculous twists just put viewers off.
BBC/John Sullivam creating the perfect ending for the trotters, only to ruin the legacy with 3 pointless unfunny episodes where they lost all their money. travesty
I've always kind of disagreed with this, I admit the first comeback episode was poor but for me the 2nd one (strangers on the shore?) is one of the best episodes & the last episode wasn't the greatest but was a nice round off
Tho I do think that the '96 christmas trio are 3 of the greatest sitcom episodes ever made

I used to love that show. The Oliver Reed episode was an absolute classic.
Torchwood having a co-production deal with a US network.
The show had gradually improved with each series until we hit the third series, Children of Earth (featuring a certain Peter Capaldi), which was one of the best examples of event TV and sci-fi in a long while. The show had never been better.
Then the US-based fourth series Miracle Day came along and turned the show into the exact thing it had mocked since its start, and all but destroyed the show in the space of ten episodes.
The amateur and dreadful production of the Queen's Golden Jubilee by the BBC.
This - and then the BBC defending their coverage of the water pageant rather than putting their hands up and admitting they had made a huge miscalculation.
One that made me chuckle was when Neighbours had to recast the role of Cheryl Stark for a while, and used the actress many of us remembered as Doreen Anderson in Prisoner Cell Block H - I just couldn't take that one seriously!
Masterchef not choosing Digger Dean as the second winner! Still not happy about Peter winning, and it was 8 years ago!
On US TV - Fox not greenlighting the Glee spin-off series that Ryan Murphy wanted for the graduates (Rachel, Kurt and co) and so allowing the main show to continue to be about a Glee Club in high school. Their indecision, and subsequent lack of focus, killed the show's potential, and has forced some ridiculous storyline decisions over the years since then.
Also on US TV, Toby's fate being indicted for leaking classified information in the final season of The West Wing - hated, hated what they did to his character. It destroyed so much of what they'd painted as Toby's MO. Richard Schiff, who played him, is still pissed off about it all these years later too!

Channel 5 giving up on Breaking Bad after 2 seasons... Most folk had no idea they showed it, including me at the time.

But surely it couldn't have been worse than their coverage of the Diamond Jubilee.
Having the terminally irritating Rhod Gilbert as the permanent host of Buzzcocks.
Replacing Bill Bailey/ Sean Hughes instead of Phil Jupitus.

On the first episode of the 2015 Bake Off, the contestants must impress guest judge Thrasymachus by making a delicious Humble Pie!:)
