I'm going to say that the answer to question one is because she didn't properly have her powers, and the answer to question two is either that Mordred was too weak to properly project at that point, or that Merlin was too far away (we don't know he was in the castle when Mordred said Morgana's name). But I'm just filling in the plot holes.
Although I thought it was implied Merlin could hear Kara when she spoke to Mordred in series 5, so it would seem that mind conversations are not private.
