Mass Effect confusion

Hey, I'm playing my third playthrough of Mass Effect. Two of the playthroughs, including this one, I have maxed out my paragon bar. However, I have not got the achievement for accumulating 75% paragon points or been given the assignment that you get when you reach 75%.

Why haven't I got the acheivement? Does it not mean 75% of the bar? Does it actually mean 75% of the total paragon points you can be awarded?

Edit: Actually I have just been given the assignment when I just started playing it again but I still don't have the achievement.

    It's 75% of the bar.

    Well my Xbox has mucked up then. I think its to do with the fact that my first playtrough I never had an internet connection, and when I finally got it, I found I had to recover my gamertag so that playthrough never existed.

    I have gotten most of the achievements I had earned through the first playthrough except that one.
