It's been pointed out at Gallifrey Base that this is just a microphone that was either not concealed properly or that somehow evaded being eradicated in the final cut of the episode... just a production error

Still, it didn't stop people speculating that it was a midget silent hopping on the spot behind the guy's shoulder

Was only watching an SD version so couldn't be sure.
It seemed to me to be a strange position to be using a boom on a closed studio set, that's all. I guess they didn't have any suitable alts to use in the edit.
