I read somewhere that Dorium was meant to be a hero in A Good Man Goes To War when in reality he was a coward.

If Dorium was blue guy (not so good with names) then he did go towards the headless ones despite being told not to. That was pretty brave, could be a hero because of that?

He was a good guy, and he went bad when his head fell off.

Why didn't Doriums head get replaced by a pig tail like everyone elses? How did the hoods stay up? Where is the real baby?

He was pressganged into heroics, and tried to negotiate his way out of the fight.

He didn't have a twisted knot thing on his neck because he wasn't converted, he was simply killed. He kept walking because - frankly - that kind of stuff creeps kids out, which really is the aim of the game.

He was pressganged no doubt but trying to negotiate your way out of a fight isn't cowardly, it's just common sense.

We'll find out exactly what it was he owed the Doc when he next appears and we'll probably be presented with a moral question like 'was his debt worth his life'.
They've been very heavy on how the doctors been realising his intentions as of late and I wouldnt be suprised if there was some sort of retrospective on people who have died fighting his causes etc.

As for Dorium being a coward...I'd call him a nicely realised anti hero along with the silurian and sontaran.

The whole concept of the Headless Monks, and why they recruit, hasn't been explained - so perhaps there is something more behind it. Perhaps his head is just temporally phase-shifted or something.

I just assumed that tying that twisted knot thing took time, probably involved a good bit of ritual and was to make it neat rather than a key component.

As they were entering combat they just didn't have the time, so skipped it

Actually, having pored over it a few times, there is a distinct pyramidal shape that suggests they have in fact done the twisty neck thing:
