Could he have told her when The Doctor "died" in LKH? It does make sense when she says "I'm sure she already knows." (sorry if already mentioned)

at the end didn't River tell Amy that the Dr did not tell her his name as she lied about that.

At Amy and Rory's wedding when the Doctor calls Rory "Rory Pond", Rory replies "it doesn't work like that." Maybe it does work like that on Gallifrey. So the Doctor would take his wife's surname, thus being Dr Song (previously being Dr Monroe after marrying Marilyn and Dr The First when he married Elizabeth the First).
