I had a few problems at first but the image size i use for the kindle is 510px by 680px. You dont have to insert a cover into the docx file i dont think, you can insert it in calibre, browse for file and then click trim to fit, seems to work, the cover appears as the first page.

Also if your wanting to change paragraph spaces or layout things then i find it best to convert to epub, zip it up then extract the files and then you can work on the html and css in a program like Dreamweaver if your clued up on website design etc.
