Think they lost it with the yellow card - JH seems to be a bit of a liability in this respect. Three enforced changes may also have had an effect.
Didn't see it as I was listening to it on the radio, but I heard they were being penalised for the front row engaging too early, nothing to do with the number eight. It's something they got hit with repeatedly in the six nations so looks like they've not learned from that.
