Has anyone ever had a issue where itunes has taken money out of your bank for items bought via a app that you never authorised ?

If so how did you go about getting this resolved ?

Look at your Purchase History in your iTunes account via the iTunes Store and there is a button at the bottom called Report a Problem.
Not happened to me, but seen several reports before on the AVForums, usually for the same app too.

Can you clarify what you mean when you say you 'never authorised' the transactions?

When you attempt to purchase something inside an app it will pop up a confirmation box that says 'Confim Your In App Purchase' that tells you what you are buying and the price, with cancel and buy buttons. This cannot be bypassed by apps - a finger tap on 'Buy' is required to authorise an in app purchase. Your iTunes account password will also be required, unless you made an in app purchase within the last 15 minutes.

Do you mean that an app is somehow bypassing this confirmation step and making purchases without your knowledge or consent, or do you mean that someone else is making in app purchases on a device using your iTunes account?

Here is one for light reading;
AVForums

Some people have stated their iTunes account was hacked.
If you are saying you have been charged for purchases you didn't make then I suggest you change your iTunes password asap and report to Apple.

Only time I've had to do something similar is when an in app add on failed to download. Emailed apple and they refunded me.

Thought ild lost the money to be honest so was surprised with how effortless and quick sorting it out was.
