But Madonna hasn't lost her fame. She may not get big chart success with singles, but she is the subject of umpteen threads, tweets and media reports. No other celebrity is getting as much media attention as she is at the moment. Madonna is slightly egotistical (not a criticism - she has to be to have survived in the business for as long as she has -), in her eyes she has not fallen from the throne, is still leading the way and has plenty to achieve. It is highly unlikely she would make a record about her losing pop's most significant crown. Yes, she has sung about her dislikes in the business and her desires to be left alone and the questioning of herself had she decided to trade it all in, but "Queen" is an apocalyptic song. Not a personal song. But, your interpretation of it proves exactly how ambiguous the lyrics are. The inclusion of the terms CIA and MI6 indicate a major crisis.

I think the "Queen" in the song is symbolically representing all world leaders. Not particularly about a singular ruler but collective.
She has said also regarding Ghosttown that she wrote a song about the apocalypse but then started wondering what would happen afterwards. What would happen if just a few survived? That is what Ghosttown is about.
