The way I took what 11 said, is that Hurt is 0. The person that once he regenerated into 1(Hartnell), became the Doctor and ran from Gallifrey and who he was before he was "The Doctor"
I think I have worked out who John Hurt is OMG, and it makes sense too.

If John Hurt's character has broke the promise then the promise has already been made to be the doctor. Therefore it cant be zero!
The Watcher was between the Fourth and Fifth Doctors and the Valeyard will be between the Twelfth and Thirteenth. They are "interim Doctors" if you like.
I believe John Hurt's character is another "interim Doctor", but this time between the Eighth and Ninth, and as with the Watcher and Valeyard doesn't affect the numbering system.
