If therapists are acceptable, two recent-ish films:
The Diving Bell & The Butterfly
The Sessions

My Left Foot.

A bit of a stretch, but:

Never Let Me Go
The Caretakers (Joan Crawford and Robert Flack).

Think there must be quite a few but the carer/support worker isn't always acknowledged as such. (The brave self sacrificing relative just as one example )

Will toddle off and think about this, just remembered Benny and Joon where the brother cared for Joon.

Also seeing Joan Crawford's name made me think of 'Whatever happened to Baby Jane?'

Bette Davis in a frilly frock and scarey - nay! - terrifying make up, looking after me, would, without a doubt produce a fatal heart attack

Untouchable - a French film from a couple of years back about a paraplegic rich man who hires a young man from the projects to be his carer.
