With respect Live +3 ratings don't matter at all. Unless they can devise a way where people don't skip the ads, they're meaningless.

They do for a network like FOX, who are taking them into consideration

and it is interesting to see what shows are doing well through DVR.

As little-monster said, Fox takes them into consideration.
They would also be referred to when making product placement deals, which is becoming a more common way of a programme generating revenue... so although they aren't the primary indication as to how profitable a programme can be, they do have their uses.
Plus they are interesting.

General consensus is that it only got a renewal because it was in its 3rd season and just needed one more to make it an appealable syndication package for CBS to sell. Many thought they were going to move it to Friday nights and cancel either Hawaii Five-0 or Blue Bloods to make space for it, but both are consistent performs by Friday standards so they both got renewed and Elementary stayed put. Until now when CBS have clearly lost faith and are going to burn off the final episodes in a Sunday night graveyard slot.

It wouldn't shock me if Elementary got a short fifth season. It all depends how the writers have chosen to end the season. If they have kept an eye on the ratings (which all shows should do as the network informs them), then it would be wise for them to film this season finale as a wrap up.

The first two seasons were really good. But then something happened during the 3rd season and it became really dull and depressing, and Lucy Lui wasn't in it as much.

By this last season the damage had been done.
