I would guess (but may be wrong) that if David does die (and we don't know if he does yet) it would have been a joint decision between Michael and Lorraine/DTC. Michael only ever intended to come back for a short stint and probably did so to give the character closure.

If he dies he'll do so as a redeemed character who DID love Carol. She'll find out soon enough he was rushing to meet her. Therefore her heart is broken, but she'll know he loved her.

It's a shame but I think since his return his character has only worked really well with Janine's. I didn't buy the Carol stuff as it's all made up bull and he didn't spend enough time with Ian.
