Buying T-Shirts online - Sizes?

I'm looking at T-Shirts and not sure about sizes. If it says on their site that medium is 38-40, is that inches around the chest area, and also will that be the size of the T-Shirt or the size of the chest that it fits. So if my chest is 40 inches in circumference will that mean the shirt is tight around my chest. The next size up, large, is 42-44 and I want some bagginess around my chest. Should I go for large or medium?

Most t shirts shrink in the wash so perhaps buy a size larger if you are buying cheap.

Thats what I would have thought, although I wouldn't say they're cheap. £35 for the most expensive. I'd like to avoid having to return them but I guess that's the perils of buying clothes online.
