Surely this has been covered?
As it happens, there was only a period of less than 2 months between the start of the first Pope John Paul's time and the next. Unless the question was asked in this small window there were 2 so the first one is known as The First. Referring to Victoria as V1 does not make sense as there might never be another Queen Victoria. If there is (but only if) then she will be called Victoria The First from that point. It's not about what someone was called during their reign (or equivalent) but what they are known as now, or when the question was asked. This is just the way the system works. Even the car example is the same. Now it would be called Mk1. If you are being asked now it means the extra detail is included.
