No HM has ever been thrown out for bullying, unfortunately.

But some certainly have been thrown out for temporary things, such as Denis for spitting on Mo in bb9. He didn't regularly go around spitting. It could be considered a temporary loss of control. But he was still thrown out, and rightly so.

Austin should at least have received a strong formal warning of the sort Janice received earlier in this CBB.

BTW, didn't Austin receive a warning then too?
