Yes, she did not get exterminated. She was given something by Missy that used the Dalek energy from the extermination blasts to send her away - as explained in the episode with a 3 or 4 minute cutaway, really detailing how it had happened. Presumably to avoid the question of how it was done...
The point I was trying to make is that whenever she seems to have died before, there was a reason why swhe turned out not be dead after all. Whilst there is no doubt she is leaving, she doesn't have to be dead. The last companion that was actually killed off was Adric and he was really unpopular with fans. Plus his death was more heroic. Clara only took the brand because she thought she would not be harmed so not so heroic as made out.

I have to disagree. The most trsgic death was katarina sacrificing herself to save the first doctor, bret and stephen in the daleks masterplan.

As for Americanisms, these are in fact Britishisms. Don't believe me, then ponder where their language came from, where their people came from (mostly) The same is said for spelling, just ask Shakespear. That very British author and play-write wrote his surname in several different ways.
As for the show, well the TARDIS translation circuit decides what you will actually hear, so I don't know why anyone would get their knickers/panties/pants in a bunch/twist/knot
