In 'The Five Doctors' when Rassilon spoke it seems only the first Doctor recognised his voice. Could that indicate that 'The Other' was the Doctor, as hinted at in the McCoy era?
The first Doctor & Rassilon
I think they've broke away from that now, as it wasn't a very popular idea with everyone to have the Doctor such a powerful figure rather than the wandering traveller. It's pretty much just part of the Virgin New Adventures continuity now, I think.
Its all retroactive though. The concept of timelords was not introduced until end of 2nd Doctor regime. Mind you, with time travel anything is possible.

No. In 1983 the idea of "The Other" hadn't even been come up with. I imagine the people responsible were still swotting for their A Levels and reading 2000AD. Or they were on BBC courses for bourgeois right-on kids wanting to be screenwriters while reading 2000AD, one or the other.
If the classic series had continued, we'd know by now (or have a very good idea) that the Doctor was The Other and had a personal history with Rassilon and Omega.
But then, if the TV movie had continued into a series, we'd know by now that the Doctor was in fact half Time Lord and half Atlantean.
The Doctor's history remains fluid, entirely dependent on who's writing the show at any given time. We’ve even had an additional incarnation sneaked in at the last minute. Who knew?
But pretty much anything else is still possible too.
• The Doctor and Missy could still be revealed as siblings.
• The Doctor might still be revealed as The Other.
• Claire Bloom's "The Woman" could still be revealed as the Doctor's mum. Or Ace. Or a future Rose.
• Omega could still be revealed as the Doctor's husband.
• Even when something is revealed (e.g. the Doctor being half-human) it can be retrospectively erased. Which is what appears to have happened.
With Doctor Who, anything could happen. And probably will.

Well it's said wisdom comes with age, so shouldn't the 5th doctor have been the wisest at that point. He has all of the others knowledge and wisdom and then some on top to boot. The first doctor, being the youngest and having experienced the least in his life out of all of them would logically be the least wise.
Just how, by the same logic, 12 would now be more wise and knowledgeable than in any incarnation beforehand. Just like how you as a person would be slightly wiser and more knowledgeable than you were a year/five years/ten years ago.

I was talking about the general mythos of the programme. But don't let that stop you trying to score points by trying to sound smart. I know 'The Other' hadn't been thought of, as Andrew Cartmel had not by that point joined the production staff, but is it possible he saw that episode and had the idea of retconning the idea into the shows history?

I was "trying" to express my disdain for everything to do with Andrew Cartmel's tenure as script editor, as it so happens, particularly his so-called "masterplan". I don't care if it scores any points or not.

I'm more interested in what Terrance Dicks had in mind when he wrote the story than how Andrew Cartmel chose to interpret it later. I don't like retcons unless they're really fantastically clever and well thought-out, and in this case if Terrance Dicks didn't intend for the Doctor and Rassilon to be acquainted from ages back then I feel it shouldn't be read into the story.

Looking at the transcript (can't remember how the scene plays out on screen) it sounds like First wasn't really the first to work it out, just the only one to offer the information. That is as likely to be coincidence as anything else, he might just have got in there before the others spoke.

Well he was the only one to figure out the meaning of the message and what was actually happening.
