Aha, so what about the possibility that it is a new character entirely! I timelord/lady no one has met before but the doctor assumed died in the time war. After all, as you say, he is often saying that he is the only one ( a bit like in old films when they say, "surely nothing can go wrong now....") and the doctor has also said that he 'doesnt do domestic' several times so what if the time lord is a relative, like an evil brother! (AKA as bad sheep (wolf) of the family). It could happen.
