Where can I buy one of these?

I had one of these phono to s-video convertors supplied with my TV card to enable me to record from line sources.

However, after many a time of attempting to re connect the s video connector back into the TV card after it came out (it had a habit of coming out often due to the shortness of the cable attached to it), the pins are bent beyond belief and as a consequence, the thing will no longer fit into the back of the TV card.

Does anyone know where I can get one of these things, or is anyone willing to send one to me that they have spare and don't want?

PS - mine had four pins in it.
