James had Kidney Cancer and that can cause breathing problems so the inhaler could well be something to do with that.
He was coughing badly on LF, like he had a chest infection. Could be why he has gone a bit quiet, if he's unwell.

Coughing like he had a bit of a cough maybe. If he had a chest infection he'd be wrapped up indoors, shivering and struggling with every breath.

Not necessarily. I get regular chest infections and once the antibiotics are started, despite feeling awful, you can still function on an every day level.
