It was a secure psychiatric unit / hospital, it had security and a barrier before the entrance and it's not unreasonable to believe that there were few or even no visitors / staff (and no casual passers-by) using that particular entrance that were observant enough to spot a folded note to the side of the path.
Even so, I'm with the person who commented that Stella would have gone down to look for the note immediately and before her discussion with the doctor - though nipping down to see what Spector had thrown away would have presumably taken a while - to get in and out through the security routines.

This is picky of me but I think it's an overwhelming infatuation rather than love. Plenty of teenagers get infatuated by someone unsuitable but normally it's the leader of the local biker gang or teenage criminal from the dark side... fortunately not many teenagers run into a charismatic serial killer while babysitting his kids.
But does Katie believe he is the killer? I can't remember. I think she believes he's misunderstood and innocent and needs her belief to save him... the infatuation would be killed stone dead if she saw the monster he is.
