Yes they can - each reference can be interpreted "differently" so they can all be "brushed away" if that is deemed to be the right option. And you are making the bizarre assumption that every reference will be immediately picked upon by the innocent child who knows nothing about sex whatsoever, and yet still wants to discuss in great depth a couple of lines on a TV show.
There is no cumulative interpretation for a 7 year old child if each question they raise can "brushed aside". An innocent will not perceive some over-arching theme of sexuality if they don't understand that any of the individual lines have a sexual connotation.
The clear issue is that a "complete innocent" would not see a sexual thread running throughout the show. I suggest that you fail to understand that a "complete innocent" would not perceive the show in the way that you do (I'm not a complete innocent and I didn't perceive the "butcher exchange" in the way that you did) and so the consequences you fear will simply not arise.

As a responsible parent you appreciate that there is a duality of interpretation and you judge your response accordingly.
If your child is a complete innocent and asks why the man gave rose the wire-cutters, the responsible parent would not necessarily feel compelled to say "well she's blackmailing him about his gay love romps with the "Mr Porky" the butcher.
After reading this discussion it seems like the episode should have been called "The Doctor Minces"!
