I assume the 45 minutes is purely to make it viable for foreign markets.
No one, especially the US would want a show that is anything other than 45 mins (1 hour with ads) or 22 (30 mins with ads).

I never thought of that but Eastenders is shown in foreign markets and it lasts more than 22 mins

Hustle and Spooks seem to be rather popular in the States and their running time is longer than Who or can be cut down to suit a particular time slot as they are.

I see. Instead of the companion and the Doc working out stuff together, with the occasional spark of cheesy wit between them - you want the Doc to work out everything and the assistant just to stand there nodding and asking questions???

They do a bit of both in the new series, depending on the episode/situation.
