revolting. how could she not tell
just found this its disgusting how could she not tell it was in there

It's easily done. Usually it happens when women forget to take the tampon out at the end of their period, or they forget they already have one in and put in a second without realising.
As for why this happens, you have to remember that inserted properly, most women will not be able to feel the tampon inside them. Often, the tampon will be changed in the middle of the night when the woman is likely to be half asleep and not exactly with it. And also, internally the vaginal passage is not a straight tube, it is elasticated pocket that's wider than it is tall, so it is therefore quite easy for a tampon to become twisted sideways, compressed and/or pushed further back without being aware that this has happened. |
