DLA Form Filling Help

Well looks like i shall be reapplying again in the future due to me messing it up this time round due to the complicated form that it is

But on the boxes where it says how often, what do you put in there because i do not understand it

how often you would need that help or have those difficulties doing it for yourself.

Your not really making yourself clear. Maybe thats just me, but I don't have a clue what your talking about. If you look on the form there should be a number you can phone if you are having difficulties filling in the form. Try asking the question again if you can't be bothered phoning, but try make yourself clear so posters can help you.

a example, i need need encouraging or reminding to wash, bathe or shower. a box next to it which i should think is a tick if it applies to you then anothjer blank box slightly wider with how often above it.

This is the new form that has been designed.

Well, I thought you meant DVLA. Sorry. Well, I would put in the 'how often' box, daily or weekly. You obviously need to wash everyday so I would put daily for that box. Good luck, I hope it's all successful.

i have the form in front of me.
its asking how often you need that encouragement and for how long you would need the encouragment. would you need someone to encourage you every day? how many times a day?

with reference to filling in DLA forms - the 'how often' means how many times a day or week do you need the help and for how long each time.

I have been trained to fill in these forms so if you would like me to PM you with more specific help - I would be glad to.

Thats two different boxes if you look at the actual picture. Are you sure you are trained? One box says How often which means how many times, then another box wich says How long each time. Two different boxes.

i can see how it's confusing and the form seems to have changed in the past few months (assuming your form is up to date).

you just need to say how many time per day you need encouragement to wash etc. it may be that you need active cajoaling once a day otherwise you would not wash. it all depends how often you require such active intervention. you don't actually have to be receiving this encouragement; it may be that you don't and you don't take care of yourself.

I am in the middle of a dispute on my latest claim because i underestimated myself when i filled it in, i got the letter saying i was varied and they thought i dont need to help but i went down the the welfare and thay wote off a letter to reconsider it.

I got a letter 2 weeks ago saying i was entitled to it but only for another 2 weeks period before it stopped again.

I rang the dla up today to explain what the bit at the bottom says and they even said it themselves it was confusing

i told them i have gone worse and they said send in the extra evidence what you have been like and we will add it to what you have already and reconsider the decision.

They are also sending me an appeal form just incase it goes that far, that will just tear me to pieces as if im not feeling on the brink already.

I will not be able to claim for 3 months again if i don't get it turned over, they are messing me about and all the time this is making me feel i want to finish myself off

everyone knows that i am very ill and need all the support i can get but putting me through this is just hell.

I have already had 2 attempted suicidal overdoses earlier this year alone and again 2 years ago and i still get very bad black thoughts.

it's that bad now that i got the council to fill in my council tax benefit form as i cant do it

Yeah, it's shocking, I know someone who had a bad leg and never got it. It's just their choice and I think it's terrible. You need to have no head and no limbs to get it these days! I hope it works out for you, if your stuck with any other forms, just post again and I'm sure someone will be able to help.

sexbomb: these forms are a complete nightmare and often the decision making doesn't add up. i think you would benefit from some help in dealing with this situation.

purplecatz: can you suggest somewhere with competent people who could assist sexbomb?

the dla told me it is computer generated and it throws out the machine if i can get it or not.

i am really worried now, i'm in a biggish debt that im trying to pay off i only get the incapacity payment currently as it stands but i even got the income support stopped as well that i could get a little off.

With the bills i have to pay out it is going to be hard what with the high fuel bills and i can't even eat a proper healthy meal because i don't have the money to get it. that has helped me a lot but this is shocking to this year and im just in pieces waiting to see what happens next when i hear off them when my latest letter gets posted tomorrow

These forms a re a nightmare. My OH has had 3 heart attacks, is diabetic and hard of hearing but still didn't get it.

He mainly wants it for the parking badge not for the money, oh well

it may be that you are not capable of filling out these forms right now as you weren't able to do the council tax forms. seek assistance. not from online folk, but from someone trained in completing dla forms on behalf of people with mental health problems.

if no-one in this thread can point you to some place that can help, ask your GP to help you find somewhere.

the helpline is not the best place. these forms are very very hard. they have to be filled out in just the right way to get the money people are entitled to.

you're right to be worried about that. but i hope sexbomb will manage to find someone to help.

i object to how hard these forms are to fill out particularly considering that many needing this allowance will not have the capabilities to make their application in such a way that they get what they are, in fact, entitled to.

Then just apply to your local council, who are the ones that actually issue the Blue Badges. Having DLA makes it easier, but you can still get one without being in receipt of DLA.

As to the DLA forms, the best advice you can have is to fill out the forms stating what you life is like at its worst time. In my case I applied 5 times, the last time after being told this nugget of information I got it.

So I basically had to tell them that I can hardly walk 20 yard without getting distressed, dizzy, palpitations, out of breath and dead feeling in my legs. I also can't be left alone to cook as I have now had 2 TIA's (mini strokes in 20 years). I also don't work now as I can no longer last more then 20 minutes on my feet and get tired really easy.

Its not really lying to them, it telling them that your condition is such that it has a detrimentally and debilitating effects on your normal life.

I cannot think about how i used to feel back in my younger days and i wish i could go back like that but all my mind is set on is how ill i feel and nothing else. i worked very hard years ago and did the overtime where i could get it as i was always asking to get more hours. so i am not workshy but i don't have any energy to do it anymore

but part of me working long hours may have contributed partly to how i am now and other factors in the past how i was bullied, losing my real mum, burning the candle at both ends and endiong up getting severly depressed as i cannot cope with things now and cannot get motivated. it's not that i don't want to but my body and how i feel is not letting me.

I am getting bullied now off a neighbour saying there's nothing wrong with you, how dare he

I have been told it will be a long time before i ever feel like working again. I am a genuine case and people know that i am not sponging off the state just for the sake of it.

I just want to die as i cannot cope with my feeling of health anymore so that's why i am getting horrid thoughts. the primary care unit says i need to be around people with the same situation and share the info but i can't even get myself out to even attempt that.

it's frightening

(((((((sexbomb))))))) do you have a friend you could phone and ask them to help you go through the form, sometimes it just helps to have someone around while doing those forms, cos they are so annoying. I hope you manage to get it all sorted.
