The wasp has just stung Donna in the mouth and she's desparately trying to get The Doctor to suck it out.

It was a last resort as the sting was deadlocked.

Just an idea...

in the series 4 trailer, there is a scene where the Doctor has smoke coming out of his mouth. Maybe Donna is trying to suck it out?
