It's never confirmed but in my opinion Jack.*will* grow to become the Face of Boe. But its best left to our imaginations..:)
Actually it was confirmed. In "Last of the Timelords" Jack talks to the Doctor and Martha about how he became the Face of Boe in an ad campaign. The assumption is that he adopted that name again much later in his timeline...
RTD confirmed that was what he meant in the commentary for Last Of The Time Lords.
It makes sense - how else would The Face of Boe know about the Master and the right moment to tell the Doctor? also given he is going o live an extraordinarily long life it makes sense that his body would change and bits wouldn't last as long as other bits.

That scene could just as easly be Jack playing a joke on the Doctor and Martha after overhearing them talk about the FoB in Utopia.

But why would his body change in less than 200,000 years and then stay the same for 5 Billion years?
Howcome this new body of his can have children or are we to believe that part of the change was that he grew new reproductive organs out of his head after the previous one dropped off?

Literally anything could happen to cause the change - after all, he very nearly died recently, after being turned mortal again (after we were told that he was a fixed point in time, and will never die).
He also commented about his hair going grey in one of his adventures - therefore, his body is still ageing. He might have to spend a couple of revolutions going forward and backward through time...
I think it is best we choose for ourselves what we believe with him, as being sci-fi, there's no reason at all why he can't become pregnant, or the Face of Boe.

He could be winding the Doctor up, literally getting the last word, or he could be just saying that's what he was called when he was younger, with or without knowing there was a Face of Boe 'out there' in the future. There's so much about Jack you can't take on face value and you always get the impression his wildest stories are probably true. But might not be:)
True, but there must surely be consequences of living to be billions of years old in a body that's not designed for it. Particularly when you lead a dangerous life like Jack.

But that's not confirmation! It's like you said, an asssumption. Despite his immortality does he have any reason to believe he's going to become a big old head in a glass box? Does he even know who or what the Face of Boe is?
Perhaps the real joke is that RTD purposely left it vague enough for all the "ming mongs" (I find that term somewhat offensive actually) to begin fiercely debating the point. As I said, I think the weight of evidence suggests that somehow Jack becomes the Face of Boe millions of years from his present. It's probably also pokes a bit of fun at Barrowman and his good looks.

I think it is he who has the Boemina children. As to how that happens, I can only guess, but I'm sure a lot of "head" is involved.
As to how Jack ends up as a giant head in a jar. He could just evolve that way over millions of years. We know he gets the odd gray hair or bit of a wrinkle. That was after a couple of thousand years or something, so he does change.
I just realised I can't remember something about Torchwoof. Maybe we didn't get an answer, but what happened to his brother in the end? Was he the top big boss mastermind behind trying to get revenge on him, or was there another boss above him? I think I remember thinking there must be another boss somewhere who was behind it all, but that whole story just fizzled out I think. Then next thing we see on Torchwood is the children of earth (excellent story btw). The whole backstory with them living on some colony that was attacked was very interesting and I wish we could see more of that. Didn't they live in the same place where the master was found? The Silver Devastation? Or that was where they were from or something?
There's probably answers and I just can't remember. I will have to get round to watching that series again. Probably take months to get round to that though:) So much to catch up on.
It wasn't entirely clear but my impression is that Grey was the Big Bad and he got frozen in the torchwood cryogenic chambers, which presumably got blown up in Children of Earth. I think the confusion come because the actor playing Grey wasn't that good in the role, although given he was in scenes with John and James who are enormous personalities its not that surprising.
I don't watch Torchwood; has he ever e.g. lost a finger and grown a new one? I know all his bits sort of joined up again in Children of Earth but I guess the issue there was that otherwise he'd be dead, which obviously isn't the case here.

He heals from gunshot wounds - and they remove skin with them ( the exit holes are messy...), so he does grow bits back.

What would it take for you to believe? The Face of Boe turns to camera and says "I used to be Jack Harkness"?
